How to Obtain Birth Certificates in New York
Often you will need a copy of your birth certificate to verify your identity for a new job, joining the military, applying for a driver’s license or passport or any other legal reasons.
If you were born in New York you could obtain a certified copy of yours by following the process below:
Obtaining a copy of your birth certificate in New York is a little different because New York City is considered separate and different than New York State.
If you were born in New York City, then you will need to go through the New York City Department of Health and Mental Hygiene.
You can order online (below), or you can visit them in person at 125 Worth Street in Lower Manhattan, room 144. You must bring with you a completed application, photo ID and your fee. They are open Monday through Friday from 9:00 a.m. until 3:30 p.m.
You may also choose to mail in your application. Send it with a stamped, self-addressed envelope to:

If you were born in New York City, you must pay a non-refundable, $15 fee for each copy plus $2.75 for identity verification. You can make your check or money order out to NYC Department of Health and Mental Hygiene.
If you were born in New York State, then you must pay a $30 non-refundable fee for each copy. Make checks or money orders out to New York State Department of Health.
